Bells Web App Release - 2026.04.23

*Installs to production 4/30/2026

Summary

  • The patient card banner now displays the patient's personal pronouns in areas where additional data is shown.
  • Updated the relationship list that is presented for Patient Signatures and Bells Virtual Scribe to include a new relationship for Aunt.
  • The "Additional Details" dropdown at Session Information (Alternate Facility Location) is now configurable per agency. 
  • Users can now deselect the primary note type on Group Details.
  • The Authorizations validation now checks both Remaining Visits and Remaining Units when determining whether a patient has active authorizations.

Patients

  • The patient card banner now displays the patient's personal pronouns in areas where additional data is shown. Hovering over the pronouns displays the full extended label in a tooltip. 

Progress Notes

  • Updated the relationship list that is presented for Patient Signatures and Bells Virtual Scribe to include a new relationship for Aunt.
  • The "Additional Details" dropdown at Session Information (Alternate Facility Location) is now configurable per agency. Agencies can now control which location codes trigger the facility address requirement, preventing unwanted mandatory fields from appearing on their notes.

Group Notes

  • Users can now deselect the primary note type on Group Details. When starting a group note without a primary note type, Bells will prompt the user to select a group note type.

Validations

  • The Authorizations validation now checks both Remaining Visits and Remaining Units when determining whether a patient has active authorizations. Previously, only Remaining Visits was evaluated, which caused notes to be blocked from signing even when the patient had available authorization units. A patient is now considered to have no available authorizations only when both Remaining Visits and Remaining Units are exactly 0.

User Management 

  • The Users Management export (via Bulk Actions) now includes two new columns for Quality Assurance and Bells Scribe Enabled roles. 

 

  • Addressed an issue where the Custom Note Section duration calculation did not properly enforce matching date/time types when the first selected question was Patient DOB or Session Start/End Time. The system now correctly disables mismatched field types (e.g., selecting Patient DOB as the first question will only allow Date fields for the second question).
  • Fixed a bug where pasting text containing tab characters (e.g., from Microsoft Word) into Bells note fields or Scribbles introduced unexpected line breaks. Tabbed text now pastes without unwanted line feeds.
  • Corrected an issue where using "Delete All" on both the Quality Audits and Audit Review tabs did not reset the note to be re-audited. This prevented notes from being re-audited when running "Run for Timeframe." 
  • For myAvatar and myEvolv users, fixed an issue where Quality Assurance Dashboard filters (Dashboard, Reviews, Quality Audits) would incorrectly exclude imported notes when filtering by author, and where applying certain filter combinations caused previously selected program filters to be cleared.
  • For Quality Coach users, addressed an issue where the Questionnaire filter on the Audit Review tab was not properly filtering results to only show notes matching the selected questionnaire. The filter now correctly limits displayed results to the chosen questionnaire.
  • Fixed an issue where a service configured for both "Show" and "No Show" group note types would only appear in the service list for Show notes. The service now correctly displays for No Show notes as well, matching the configured note type settings.
    • This was hotfixed to Train 4/23/26.

Summary

  • The Supervision Dashboard Individuals tab metrics have been redesigned.
  • Duration Calculations in Custom Note Sections now support Date, Time, Session Start/End Time, and Patient Date of Birth as calculation inputs. 
  • Questions in Custom Note Sections can now be conditionally required based on Session Status (Show, No Show, Cancelled). 
  • Documentation Recommendation rules now include an Export button (below "Import from Library") that generates an Excel file of the configured rules.
  • Validations can now be configured to filter by Program or Location in addition to the existing filter for Service.
  • The Validations add/edit display now shows the name of the Validation type (example - Session Duration) being added/edited for clearer context when configuring Validations. 
  • Assorted bug fixes. 

Supervision Dashboard

  • The Supervision Dashboard Individuals tab metrics have been redesigned. The four summary metrics now display horizontally across the top of the dashboard in a single row, with the remaining content filling the space below for a cleaner layout.

Custom Note Sections

  • Duration Calculations in Custom Note Sections now support Date, Time, Session Start/End Time, and Patient Date of Birth as calculation inputs. When a date or time type is selected, incompatible types are automatically disabled to prevent mixing data types. This applies to Forms, Narratives, Imports Assessments and Subforms.
  • Questions in Custom Note Sections can now be conditionally required based on Session Status (Show, No Show, Cancelled). This allows organizations to configure different required fields depending on whether the patient was present, no-show, or cancelled. The conditional required dialog has been updated with improved labeling and a new session status section.

Documentation Recommendations

  • Documentation Recommendation rules now include an Export button (below "Import from Library") that generates an Excel file of the configured rules. Supported rule types include: Discouraged Verbs, Restricted Nouns, Ambiguous Nouns, Forbidden Text, Expand Acronyms, and Redundant Much. Files are named with the rule type and date (e.g., Discouraged-Verbs-2026-04-16.xlsx).

Validations

  • Validations can now be configured to filter by Program or Location in addition to the existing filter for Service. The following Validations were updated - Session Duration, Session Increments, Prevent Session Time Overlap, No Appointment - No Note, Session Date, Session Status, Missing Information, Insurance, Age and Span Midnight. Each updated option allows for “Only applies to” the selected Program, Location or Service as-well-as "Does not apply to" functionality. Only one filter type (Service, Program or Location) can be applied for each rule of the Validation. Helper text within the validation and the card descriptions have been updated to reflect these changes. 
  • The Validations add/edit display now shows the name of the Validation type (example - Session Duration) being added/edited for clearer context when configuring Validations. 

 

  • The "Warn at Sign of Note" outcome validation popup now correctly displays when signing a note. Previously, the warning only appeared in the right-side pane but did not show the expected popup dialog.
  • Fixed a discrepancy where the Signed Notes Report and the Supervision Dashboard showed different counts of signed notes for the same date range and filters.
  • Fixed an issue where supervisor users with access to all patients were unable to open note details in the side-by-side view for Similar, Overlapping, or Audit review items. This also corrects filter behavior in reports for supervisors in organizations with Care Team Level Access turned off.
    • NOTE: This fix was rolled back on 4/24/26. It will be addressed in a future release.
  • For myAvatar users, resolved an issue where overnight diagnosis syncing from myAvatar to Bells failed, preventing staff from starting notes due to missing active programs. 
    • This was hotfixed to train and production on 4/10/26.
  • Addressed an intermittent error that occurred when starting group notes, which prevented some users from being able to start the group note.
  • For myAvatar users, corrected an issue where Group Notes with a configured No Show note type would incorrectly default to the parent group's myAvatar Note Type instead of the note type configured for the No Show status. This affected the No Show tab, individual No Show patients, and the Group Summary for No Show sessions.


 

Bells Web App Release - 2026.04.09

Summary

  • Users can now attach PDF files to Scribbles using the same workflow as adding images. Clicking an attached PDF allows the user to download the file. 
  • The Writing Assistant's "Make Longer" prompt has been updated to maintain consistent patient name references across progress note sections. 
  • The existing toggle for Configured/All Groups in the Patient section and Organization Settings has been replaced with a multi-select dropdown. 
  • Administrators can now export their organization's Drop-Ins and Expansions. 
  • The "Program Selection" validation has been renamed to "Program Service." The "Location" validation has been renamed to “Service Location.”
  • For myAvatar and myEvolv users, imported custom note sections Signature-type questions no longer support the "Conditionally Required" setting. 
  • We have added the ability to configure the Bells link domain used in emails (reports, reminders, usage notifications, etc.). 
  • Introducing the "Reviewer Dashboard" display! 
  • For Bells Virtual Scribe users, the meeting URL support has been removed from the Bells Virtual Scribe recording flow.

Scribbles 

  • Users can now attach PDF files to Scribbles using the same workflow as adding images. Clicking an attached PDF allows the user to download the file. 

Progress Notes

  • The Writing Assistant's "Make Longer" prompt has been updated to maintain consistent patient name references across progress note sections. The assistant now uses the same term (patient name or configured patient label) as the original text, preventing inconsistent switching between names and labels within a single narrative.

Groups 

  • The existing toggle for Configured/All Groups in the Patient section and Organization Settings has been replaced with a multi-select dropdown. Users can now filter by "Configured," "Not Configured," or both to view all groups.

Administrators

  • Administrators can now export their organization's Drop-Ins and Expansions. An Export button is available to the right of the Manage/Review toggle. The export includes type, name, text, created date, created-by staff, usage count, and copied-to-org-level status. The export reflects the currently active tab (Manage or Review) and only includes Drop-Ins or Expansions visible to the logged-in user.
  • The "Program Selection" validation has been renamed to "Program Service." The "Location" validation has been renamed to “Service Location.”
  • For myAvatar and myEvolv users, imported custom note sections Signature-type questions no longer support the "Conditionally Required" setting. When a Signature question's Required toggle is enabled, the Conditionally Required option is no longer available. Existing signatures with conditional required settings will be treated as "Always" required.
  • We have added the ability to configure the Bells link domain used in emails (reports, reminders, usage notifications, etc.). Please follow the Netsmart Support contact process to discuss if your organization needs this option.

Quality Assurance

  • Introducing the "Reviewer Dashboard" display! A new tab has been added to the Quality Assurance section for QA and Admin users. This dashboard provides an audit log of users performing reviews, including widgets for Issues Needing Review, Reviews breakdown (Confirmed vs. Refuted), Confirmed Reviews percentage, and top/bottom users by review volume. The existing “Audit Review” tab has been renamed to "Reviews." This functionality is available to Bells Quality Coach users as well as Bells Documentation Assistant users. 

Add-Ons

Bells Virtual Scribe

  • For Bells Virtual Scribe users, the meeting URL support has been removed from the Bells Virtual Scribe recording flow. 

Bells Quality Coach

  • For Bells Quality Coach users, users with the Supervisor role now have access to the Quality Assurance Dashboard and Reviews tabs. Supervisors can propose edits to Quality Audit answers with justification, which QA/Admin users can then accept or reject. The Reviews tab continues to function as a worklist and automatically advancing to the next issue after saving. A new "Has Proposals" toggle on the Quality Audits tab lets users filter to audits with pending proposals.
  • For Bells Quality Coach users, the hover tooltip on the Quality Assurance Dashboard's top-right metrics widget now reads "Only the Questionnaire filter affects this section's metrics" to clarify filter behavior.

 

  • For myEvolv users, fixed an issue in subform sections of Progress Notes where additional treatment information text related to a goal was cut off in the note. Text containing special characters (such as <) is now displayed in full.
  • Corrected a bug in the Note Audit Print Preview where text on the right side of the page was cut off.
  • For myEvolv users, addressed an issue where appointments cancelled in Bells were not properly syncing the cancellation status to myEvolv. 
  • Fixed a display issue where the toggle and label for conditionally required Custom Note Section questions were misaligned.

Summary

  • Removed the ability to “Evenly distribute group duration across patients” for group notes due to lack of implementation between Bells and the Care Record. 
  • Updated the Appointments without Notes report to not show for users if the organization has the agenda turned off.  
  • Enhanced the Note Type configuration to allow for filtering of the note types available to the user when starting the progress note by the patient's insurance. 
  • Added a new Custom Note Section question type for Time. 
  • Added a new Custom Note Section question type for Empty Line. 
  • Added the ability to export the Expand Acronym rules configured in Documentation Recommendations.

  • Added a new Custom Note Section question type for Empty Line. The empty line question type allows users to configure an empty line for better control over the layout of their note.

Documentation Recommendations

  • Added the ability to export the Expand Acronym rules configured in Documentation Recommendations. 
  • Enhanced the Patient Labels Documentation Recommendation with a new toggle to use the patient's preferred name. When enabled the rule will allow the note writer to refer to the patient by their preferred name. When enabled the writing assistant functionality is also updated to use the patient's preferred name.

Add-Ons

Bells Virtual Scribe

  • For Bells Virtual Scribe users, updated the AI Draft to use the patient's preferred pronouns. 

Bells Quality Coach

  • For Bells Quality Coach users, added a new filter option to the questionnaire configuration for Insurance. 
  • For Bells Quality Coach users, updated the Quality Assurance Dashboard Breakdown by Credential widget by expanding the graph horizontally to show more credentials and changing the data sort to be by the highest number of users with the credential instead of by percentage. 

 

  • For myEvolv users, corrected a bug with Medication subforms incorrectly filtering/displaying medication information.
  • Addressed a bug when starting progress notes only showing the primary group note type as available. 
    • This was hotfixed to train and production environments on 3/27/26. 
  • For myAvatar users, fixed an issue when the Bells Note Type is changed incorrectly handling the myAvatar Note Type change. 
  • Addressed issues with the Shared Group Summary functionality when the notes synced to the care record.

  • Added a new report for “Appointments Without Notes” that shows appointments that do not have an associated note. Standard filters and note visibility are included with this report along with the ability to export and email. 

Bells Administrators

  • Updated the Organization Preferences Configurations section by splitting the configurations up between application-based configurations and note-based configurations. The note-based configurations have been parsed off the configurations display to a new tab for Notes. No behavior/functionality changes have been made as part of this update, just where the configurations are located in the Organization Preferences section. 
  • Enhanced the Custom Note Section short text question type with a new format option in the right pane for Numeric. When configured the user will only be allowed to enter numeric digits in the answer on the progress note. 
  • For myAvatar users, updated Custom Note Sections with myAvatar Note Type filters at question and answer level configuration. 
  • Enhanced Documentation Recommendations Ambiguous Noun handling to retain capitalization when acronyms are configured for suggested replacements. 

Application

  • Throughout the Bells web application updated displays with clearer language when the display is empty. No functionality was changed as part of this update. 
  • Enhanced the rich text editor (long text fields that include the format/text setting menu bar) with a new option for highlighting text. This rich text editor is the one used in Scribbles, Drop-Ins and long text/narrative progress notes questions. 

Add-Ons

Bells Quality Coach

  • For Bells Quality Coach users, enhanced the Questionnaire Question weight to allow a weight of zero (0). 
  • For Bells Quality Coach users, updated the Audit Summary report to show the icon when the audit is Flagged for attention in the right pane when viewing the specific audit for the note. As part of this update, the Audit Summary export has also been updated to include the Flag for Attention details in a new column for “Needs Attention”.  

 

  • Corrected an issue when writing progress notes preventing users from accessing scribbles that were not associated to a patient. 
  • For myEvolv users, addressed a bug for patients with only one associated person (guardian, emergency contact etc) on their record in the EHR causing an issue in Bells preventing them from signing notes. 
    • This was hotfixed to train and production on 3/24/2026.
  • Fixed a bug showing notes in Bells in a draft state after the note was started from an appointment and signed but then the appointment was deleted in the EHR.
    • This was hotfixed to production on 3/20/2026.
  • Corrected a bug when copy/pasting text from a table/grid with mixed formatting showing doubled up data when pasting into the text field.

Summary

  • Added a new widget to the Patient Chart display to show the number of sessions over time. Number of sessions is the signed notes for the patient. 
  • Added a new Organization Preference to control if the Patient's Guardian information shows in the Patient Chart or not. 
  • Updated the Audit Review display filter for Audit Findings to show all results based on the Audit Findings selected. 
  • Updated the Practitioner Service Usage report to include the Patient MRN in the exported report. 
  • Updated the Supervision Dashboard - Program tab Top Performers widget to include a better sort on the Late Notes.

Patient Chart

  • Added a new widget to the Patient Chart display to show the number of sessions over time. Number of sessions is the signed notes for the patient. The widget can be adjusted by date to show the trend of sessions within the selected date range. The new widget shows after the Distribution of Services and Show rate widgets. 
  • Added a new Organization Preference to control if the Patient's Guardian information shows in the Patient Chart or not. The new configuration is on the Configurations tab of Organization Preferences. 

Reports/Dashboard

  • Updated the Audit Review display filter for Audit Findings to show all results based on the Audit Findings selected. 
  • Updated the Practitioner Service Usage report to include the Patient MRN in the exported report. 
  • Updated the Supervision Dashboard - Program tab Top Performers widget to include a better sort on the Late Notes. 

Add-Ons

Bells Quality Coach

  • For Bells Quality Coach users, enhanced the Quality Audit review display with the ability to print the audit results. The results include the note details, note content, questions from the audit and the results that show in the right pane of the audit review. 

 

  • Corrected a bug in the note print/preview incorrectly handling custom note section headers. 
  • Addressed an issue with the Bells Writing Assistant Make Longer functionality that was replacing “this writer” with inappropriate text or credentials. 
  • Fixed a bug on searchable drop-down selections causing users to have to scroll back to the selection and select it a second time. 
  • Addressed a bug in the Audit Summary report for Quality Audit results showing an editable Flag for Attention toggle and comment field. The toggle and comment no longer show on the Audit Summary report. 
    • This was hotfixed on 3/18/2026.
  • For myEvolv users, corrected an issue when editing progress note details that was clearing out the place of service. 
  • For Bells Virtual Scribe users, addressed a bug in the Meeting URL functionality marking valid URLs as invalid. 
  • For Bells Virtual Scribe users, corrected an issue that was delaying the return of Virtual Scribe results/functionality. 
    • This was hotfixed on 3/18/2026.

Summary

  • For myAvatar and myEvolv users, update the routing handling on the sign note display from a checkbox to a yes/no toggle.
  • Added the ability to start a note from Scribble that is associated to a patient. 
  • Added a new placeholder for the patient's last name to be used in drop-ins and scribbles.
  • Updated the Patient and Practitioner Service Usage Reports to include note records that do not have a session time section. 
  • Enhanced the Program display of the Supervision Dashboard Active vs Enabled Users widget to have clearer language for what the widget is showing on hover. 
  • Added a Session to Sign metric to the Program and Individuals displays of the Supervision Dashboard.

Progress Notes

  • For myAvatar and myEvolv users, update the routing handling on the sign note display from a checkbox to a yes/no toggle. The routing label has been updated to “Route to approver?”. The behavior and configuration that manages the routing flow is not changed. 
  • Added the ability to start a note from Scribble that is associated to a patient. 
  • Added a new placeholder for the patient's last name to be used in drop-ins and scribbles.

Reports and Dashboard

  • Updated the Patient and Practitioner Service Usage Reports to include note records that do not have a session time section. These notes will display as a service for the Patient or Practitioner but have zero time counted in the totals. 
  • Enhanced the Program display of the Supervision Dashboard Active vs Enabled Users widget to have clearer language for what the widget is showing on hover. 
  • Added a Session to Sign metric to the Program and Individuals displays of the Supervision Dashboard to show the average overall Session to Sign based on the which display and filters. 
  • Updated the Top Performers and Late Signers widgets on the Program display of the Supervision Dashboard to include the Session to Sign time on the View Data display. 
  • Added the Accepted Recommendations metric to the Individuals display of the Supervision Dashboard. This metric shows the percentage of accepted recommendations based on the filters in the display. 

Patient Chart

  • Enhanced the Patient Chart right pane sidebar to allow users to customize the sections that show and the order of the sections for the patient chart display. 

Add-Ons

Bells Quality Coach

  • For Bells Quality Coach users, adjusted the placement of the “Needs Attention” button in the Quality Audit display from the left corner to under the Question display in the right pane. 

 

  • Addressed a reported issue with text fields when text was being copy/pasted causing some spacing to go missing. 
  • Corrected a bug in note header hover text for the Session Date/Time incorrectly converting the date/time due to the Daylight Savings Time change. 
  • Fixed an issue with the Clinical Pathway colors after the patient is assigned to the pathway.

Summary

  • Enhanced the patient search to be able to search on a 4 digit patient ID/MRN. 
  • Updated the Practitioner Service Usage and Patient Service Usage reports by adjusting the way the percentage shows for the Previous Period.

  • Added the ability to sort the Top Performers widget by Total Notes or Late Notes.

Patients

  • Enhanced the patient search to be able to search on a 4 digit patient ID/MRN. 

Reports and Dashboards

  • Updated the Practitioner Service Usage and Patient Service Usage reports by adjusting the way the percentage shows for the Previous Period. 
  • Enhanced all reports in the report section to include a report loading display including a cancel button and where possible the number of records being loaded for the report. This loading display will show when opening the report, changing filters or exporting as needed. 
  • Added the ability to sort the Top Performers widget by Total Notes or Late Notes.

Progress Notes

  • Added a new organization preference to control if Group Summaries are shared across all show statuses. When toggled off, the group summaries across show, no show and cancelled (where applicable) will be separate and the copy summary functionality is available. When toggled on, the group summaries across all statuses will be the same and changes to the summary will reflect in all group summaries. The copy summary functionality will not be available.

Custom Note Sections

  • In Custom Note Sections, Imports and Assessments, the Instruction question type can now be configured as patient-viewable (used with patient signatures).
  • Enhanced the Single-choice question type as part of Custom Note Sections, Imports and Assessments to include Action functionality. For each answer option in the question, an Action link is available to configure if the answer option should “Do nothing” (default option) or “Change Service Type”. When configured to change service type is configured and the answer option is selected in the note an additional field shows to select a new service for the note. 

Groups

  • The Groups tab, under Patients/Groups, now has a Recent Groups section which shows you the last 5 groups you used to write a group note.

Reports/Dashboards

  • On the Sync Issues report, the issue text will now remain after a sync issue is cleared.
  • Updated the Supervision Dashboard Program Display Top Performers and Late Signer widgets. These widgets have been updated to more accurately display users that are top performers and late signers. The late signer widget will now filter out users if their percentage is less than 50%. Both widgets' colorization based on the percentage has been updated. 

myAvatar

  • When configured to Create Appointment on note signing, do not show the Create Appointment checkbox unless the note has a start date, end date, location, place of service and episode.  These are required myAvatar to avoid a sync error.
  • Added a new toggle to the Program Service validation that allows you to limit services codes to only the services configured through the validation, based on the selected program.
  • On the Note Type section for Add-Ons, you can now set the Add-on duration to be minutes only or hh:mm.
  • On a note type, you can now turn off the ability to add an ad hoc problem in the Problem section of a note.
  • A patient's contacts in Bells now includes contacts entered via myAvatar's Emergency Contact form.
  • A nightly Sync Issues job will run to clear any sync issues that have been resolved outside of Bells, but not marked as resolved in Bells.
  • Introducing Clinical Pathways! Clinical Pathways now sync into Bells from myAvatar and Bells Pathways can be configured in Bells to enroll patients for the pathway. Several areas in Bells have been updated to support the new pathways including Custom Note Sections/Imports/Assessment/Subforms questions and answer filters, Quality Coach questionnaires and Note Types. 

myEvolv

  • On a note type, you can now turn off the ability to add an ad hoc problem in the Problem section of a note.

 

Add-Ons

Bells Quality Coach

  • Added a Program filter to the Quality Audit display.
  • Added a new ‘Notes Not Audited" report to Reports to show all notes that weren’t audited based on a specific time frame, practitioner, client or the note origin.
  • For myEvolv Quality Coach users, finalized notes from myEvolv now import into Bells automatically. Once imported, these notes are read only and will flow through the Quality Coach note audits. Signed note displays in Bells have been updated to include an Origin filter to see just notes from Bells, just notes from myEvolv or notes from both. Notes that are imported from myEvolv have the import icon on the note record.

  • Added Patient View (used with patient signatures) to Assessments.
  • In group notes, fixed filtering to pull the correct services when an organization is configured with the Group Note Force Service Override preference and the Group Note Type is set to filter Services.
  • Updated remember logic to clear invalid location defaulting.
  • Updated edit session logic for individual and group notes so that all validations are checked when the session information changes.
  • In session details, if a Bells note type has been selected and then the session status changes to a status that does not apply to the note type, unselect the note type so the user can select an appropriate note type for the status.
  • Updated starting an assessment note from an appointment so that it shows the same assessment note types as when the note isn't from an appointment.
  • Corrected sorting on the Signed Notes report so that sorting by session date works appropriately.
  • Corrected the emailed version of Signed Notes report so that it matches what the exported version of the report produces.
  • Quality Coach
    • Fixed the displays to show the Bells Virtual Scribe microphone icon on sessions created via Bells Virtual Scribe.
    • Improved how expansions are handled to avoid an app error when reviewing multiple notes rapidly.
    • Corrected Quality Audit displays to show the calendar icon when the note originated from an appointment.
