EXPLANATION
The Daily Living Activities Functional assessment form allows for the online entry of the client’s scores and calculates the assessment score. As with any form it must be added to a packet before it can be used in client charts. Click the link to view the instructions for adding a form to a packet.

Assessment Tab
The Assessment tab contains the twenty standard Daily Living assessment questions with the score drop down. The score description is present in the drop down. When the score is selected the full description presents.

Results Tab
The scores are automatically calculated and presented on the results tab. The results tab also offers a text area to add recommendation and justification documentation.

Delta DLA Score equals the difference of the first “DLA 20 Score” and current “DLA 20 Score. 

Printed Report
The printed report includes each question and score along with the descriptions.
